Let’s find the right Medicare Advantage plan for you
HealthPartners UnityPoint Health (PPO) Medicare Advantage plans are a great way to get more from Medicare. These plans bring medical and prescription drug coverage together to make your benefits easier to use. You’ll also get extra perks and coverage that go beyond what’s provided by Original Medicare alone.
Plus, all of our Medicare Advantage plans have overall
Your health plan should help you live healthier, and that’s why with our plans, your needs always come first:
Our Medicare network includes an impressive selection of doctors, clinics, hospitals and specialties to cover any health care need you might have. You can also see any provider that accepts Medicare – you’ll pay the same copays and coinsurance for covered services, whether your provider is in network or out of network.
You’ll never pay a primary care copay, and you can also choose a plan where monthly premiums are $0. It’s easy to find an option that fits your budget, and no matter which plan you pick, an out-of-pocket maximum makes sure your spending never goes over a certain amount each year.
All plans feature a dental benefit (increased for 2023) that can be used for preventive and comprehensive services, like cleanings, exams, X-rays, fluoride treatment and sealants, plus periodontal maintenance (deep cleanings). There’s no deductible or waiting period.
For prescription drug coverage (Part D), copays are as low as $0 for formulary medicines in Tier 1. You’ll also get coverage for insulin and most Part D vaccines: You won’t pay a deductible or more than $35 for a one-month supply of each insulin product, and there’s also no copay or deductible for most vaccines (such as Shingrix, Tdap and MMR).
You’ll have coverage for routine hearing exams, more options for hearing aids through TruHearing® (enhanced for 2023), an annual benefit allowance for non-Medicare-covered prescription eyewear (enhanced for 2023)**, and a gym membership and home fitness kit through our partnership with SilverSneakers®. Plus, you’ll get a quarterly allowance (increased for 2023) to spend on select over-the-counter products and supplies.
Enjoying the snowbird or RV life? All plans include in-network coverage for up to nine months of annual domestic travel. You’ll also have access to pre-travel consultations and worldwide emergency and urgent care coverage, with additional assistance through Assist America®.***

Many Medicare Advantage plans also include Part D prescription drug coverage. With a Medicare Advantage plan, you get the convenience of having all your Medicare coverage in one plan.
When you enroll in a Medicare Advantage plan, you will no longer receive Parts A and B coverage through Original Medicare. Instead, the company you purchase your Medicare Advantage plan from will administer those benefits along with any additional coverage the plan offers.
You must be enrolled in Original Medicare Parts A and B before you can enroll in a Medicare Advantage plan.
- Annual Enrollment Period: October 15 to December 7 – During Medicare’s
Annual Enrollment Period(AEP), you can enroll in a Medicare Advantage plan for the first time or switch to a different plan. You can also drop your plan and go back to Original Medicare. Coverage by any new plan starts on January 1 of the next year.
- Medicare Advantage Open Enrollment Period: January 1 to March 31 – If you’re enrolled in a Medicare Advantage plan, you have an opportunity to change plans between January 1 and March 31 each year. You can switch to a different Medicare Advantage plan or go back to Original Medicare.
- Initial Enrollment Period: When you turn 65 – If you sign up for Original Medicare during your Initial Enrollment Period (IEP), you can enroll in a private Medicare plan any time after you’ve signed up for Original Medicare and are still in your IEP.
- Special Enrollment Period (SEP) – After certain events, such as moving or losing your current Medicare plan, you may qualify for an SEP that allows you to enroll in a different plan.
Every year, Medicare evaluates plans based on a 5-star rating system.
