Your personal information

“Personal Information” is information that can identify you individually, either directly or in combination with other information. For example, we may collect Personal Information such as your name, email address and employment history when you apply for a position with HealthPartners UnityPoint Health, your email address and opinions when you participate in our surveys, your demographic information when sign up for volunteer opportunities, or ask us question.

Children under the age of thirteen

The Website is not intended for minors under the age of thirteen without the permission of a parent or guardian.
