Care from a team that puts members first
When you combine care and insurance, it makes everything simple. UnityPoint Health and HealthPartners make this an option. You get great care from trusted doctors and insurance that covers what you need. We are members-focused, all the time. This means you always get the level of personal support you need.
A partnership focused on improving health and well-being
Simply put, we partner to make things easier. We work to improve health and well-being – in partnership with our members, patients and community members. Partnership means that with our Medicare plans you can continue seeing your UnityPoint Health doctors (or any in-network doctor).
Will I get the right support?
It’s true, there’s a lot to consider with Medicare. This is why getting the right type of support — when you need it — is critical. With HealthPartners UnityPoint Health, you have direct access to a team of experts. We are ready to answer questions.
We are a members-focused organization. And the HealthPartners and UnityPoint Health partnership means that we put you first, no matter what. One way we do this is by making it easy to get the right types of personal support.
With our Medicare plans, you get:
- CareLine Service℠: Find out if you need to see a doctor, ask questions about a medicine you’re taking or learn about treatment options. This is available 24/7, 365 days a year.
- HealthPartners® Nurse Navigator℠ Program: Ask questions about your care and benefits, choosing a treatment option, and more.
- Behavioral Health Navigators: Find a mental or chemical health professional in your network.
You also get support with extra perks and benefits, like coverage when you’re traveling and access to a large network of your favorite doctors.
The benefits of choosing a Medicare Advantage plan
Our Medicare Advantage plans allow you to get all your insurance in one plan, and you can continue seeing your doctors. This is a good choice if you want a plan with a lower monthly premium.
